About The Position

Coordinates case management activities with members to evaluate medical needs and facilitate their overall wellness. Develops strategies to address issues and outcomes and identify opportunities to enhance members’ overall wellness through integration. Instructs members on programs and procedures in compliance with network management and clinical coverage policies. The role requires residence in one of the specified Louisiana parishes and includes travel within 125 miles round trip for one day in the field. The work schedule is variable, with the ability to work two days weekly until 9 p.m. local time.

Requirements

• Must reside in Louisiana in one of the following Region 2 parishes: West Feliciana, East Feliciana, Pointe Coupee, West Baton Rouge, East Baton Rouge, Iberville, or Ascension.
• Current, active RN clinical licensure.
• Case management experience.
• Strong customer service orientation and problem-solving skills.
• Excellent communication and telephonic skills.
• Excellent motivational interviewing skills and ability to build rapport and trust telephonically.
• Excellent documentation skills and ability to respect compliance and regulatory standards.
• Highly organized and self-driven without direct supervision.
• Ability to manage time effectively and achieve performance metrics.
• Technology proficiency.
• Ability to travel within 125 miles round trip for one day in the field.
• Ability to work a variable schedule, including two days weekly until 9 p.m. local time.

Nice To Haves

• Bachelor’s and/or master’s degree.
• CCM certification.
• Previous experience in a managed care organization.
