About The Position

Compassionate Care Hospice, an Amedisys company, is hiring an on-site RN Clinical Manager in Clermont, Florida. The role leads a dedicated team and supports exceptional hospice care for patients in their homes. The schedule is Monday through Friday, 8:00 AM to 5:00 PM, with a flexible AOC and home-based schedule. Compensation is $95,000–$100,000 annually, with performance-based bonus opportunities. A $5,000 sign-on bonus is offered. Final compensation depends on experience, qualifications, and skills. The RN Clinical Manager organizes clinical operations, oversees patient-care quality and staff performance, and coordinates referrals, admissions, scheduling, and documentation. The manager supports care planning, staff training, and resolution of patient, family, and physician concerns, and participates in the on-call process. Infrequent direct patient care may be required during emergencies. The position requires an associate or bachelor’s degree in nursing, at least one year of clinical RN experience in home health or hospice, at least one year of RN management experience, a current RN license for the assigned state(s), and current CPR certification. Employment requires a background screening through the Florida Care Provider Background Screening Clearinghouse.

Responsibilities

Support, participate, and work cooperatively with the director of staffing.
Be responsible for the overall quality of patient care and the clinical performance of nurses and aides.
Organize clinical operations for the care center.
Review service requests and determine patient eligibility and suitability for home care services.
Work with intake to coordinate patient referrals and admissions.
Oversee scheduling.
Oversee payor verification and precertification requirements.
Review staff documentation and ensure missing, incomplete, or untimely documentation issues are resolved.
Assist clinicians in establishing immediate and long-term patient goals, setting priorities, and developing the Plan of Care (POC).
Regularly evaluate patient needs and collaborate with clinicians on necessary revisions to the plan of care.
Provide direct patient care infrequently in emergencies.
Participate in the on-call process.
Assist with staff training to ensure quality care, compliance, and fiscal responsibility.
Participate in investigating and resolving patient, family, and physician concerns.
