RN | Clinic Registered Nurse | PACE Program
About The Position
The PACE (Program of All-Inclusive Care for the Elderly) program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity. An RN Clinic Nurse in Midland Care Connection’s PACE program provides hands-on nursing care to PACE participants in the clinic setting. The RN assesses participants’ health needs, monitors changes in their conditions, administers medications and treatments, responds to urgent health concerns, and works closely with the interdisciplinary care team to coordinate services. They also educate participants and caregivers, help manage chronic conditions, communicate with providers, and ensure each participant receives safe, personalized care that supports their health and independence.
Requirements
- Current, unrestricted Registered Nurse (RN) license in the state of Kansas
- Graduate of an accredited nursing program
- Current CPR certification
- Experience providing nursing care to older adults or individuals with complex medical needs
- Strong assessment, critical-thinking, and clinical decision-making skills
- Ability to communicate effectively with participants, families, caregivers, and members of the interdisciplinary care team
- Ability to accurately document patient care and maintain confidentiality
- Ability to work collaboratively as part of an interdisciplinary team
- Commitment to providing compassionate, participant-centered care
Nice To Haves
- Experience working in PACE, geriatrics, primary care, home health, hospice, or another community-based care setting
- Experience caring for individuals with multiple chronic health conditions
- Experience working with an interdisciplinary care team
- Familiarity with electronic medical records and clinical documentation systems
- Knowledge of Medicare, Medicaid, or other programs serving older adults
- Previous experience providing education to participants and caregivers
