About The Position
Sage Group HealthCare Staffing is seeking a Home Health RN Case Manager for a contract position in Lebanon, NH. This role involves screening, evaluating, re-evaluating, and treating patients, as well as coordinating and communicating treatment activities with the interdisciplinary team, patient, and caregivers. The RN Case Manager will develop and periodically review care plans in collaboration with the patient's physician, patient, and family. They will provide ongoing assessment of the patient's clinical, functional, and emotional status, communicate significant changes to the physician and colleagues, and teach patients and caregivers self-care techniques. Medication, diet, and other instructions will be provided as ordered by the physician, with opportunities for health counseling.
Requirements
- Registered Nurse: 2 years - Required
- Home Health Nursing experience: 2 years – Required
- Must have valid Driver’s License and Personal Auto Insurance
- ASN or an equivalent – Required
- RN License for State/s applying or Compact - Required
- BLS Certification from AHA or American Red Cross - Required
Nice To Haves
- EMR Epic - Preferred
- BSN or an equivalent – Highly preferred
Responsibilities
- Screen, evaluate, re-evaluate, and treat patients.
- Coordinate and communicate treatment activities with the interdisciplinary team members and the patient.
- Develop a care plan in collaboration with the patient's physician, the patient, and their family, or the primary caregivers upon admission.
- Periodically review the patient care plan and recertify the patient care plan as appropriate.
- Coordinate the patient's care plan through interdisciplinary communication and case conferences.
- Advise other disciplines of discharges in a timely manner.
- Provide ongoing assessment of the patient's clinical, functional, and emotional status to identify problems and/or progress toward desired goals.
- Communicate significant changes to the patient's physician and colleagues.
- Document evaluations, treatment goals, and objectives.
- Teach the patient and caregiver self-care techniques as appropriate.
- Provide medication, diet, and other instructions as ordered by the physician and utilizes opportunities for health counseling with patients and caregivers.
