About The Position

At St. Croix Hospice, the RN Case Manager provides one-to-one holistic support to patients and families in their homes. The role partners with an interdisciplinary team to develop individualized care plans, guide end-of-life conversations, and promote patient comfort with professionalism and empathy. Requirements

• Associate, bachelor's, or master's degree from an accredited nursing program (RN).
• At least one year of recent acute care experience.
• Reliable transportation, a valid driver's license, and proof of auto insurance.

Nice To Haves

• Hospice or home health experience.

Responsibilities

• Complete initial and ongoing comprehensive assessments to determine hospice needs.
• Develop individualized care plans with patients and families, updating them as needs evolve.
• Administer prescribed medications and treatments.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
