About The Position

Aegis Healthcare is expanding our Home Health and Hospice Nursing Teams! As a Home Health & Hospice Nurse Case Manager, you will be instrumental in providing exceptional care to patients in their homes. Your role will include conducting comprehensive physical, social, and psychological assessments, collaborating with patients, families, and the Interdisciplinary Group to create individualized care plans, managing care delivery, and maintaining meticulous patient records. You will also act as a resource for families, making proactive referrals, and fostering a holistic care experience.

Requirements

Active Registered Nurse (RN) license in Arizona (or state of practice).
Valid driver's license, reliable transportation, and proof of current auto insurance.
Ability to perform patient transfers with a full range of movement.
Current CPR certification and negative TB test (we can assist).
Valid AZ Fingerprint Clearance Card (we can help obtain it).
All Aegis Healthcare employees must be able to pass a criminal background check.

Nice To Haves

Previous Home Health or Hospice experience.
Effective verbal, written, and time management skills.
