About The Position

Independence Home Health LLC is seeking a compassionate, dependable, and motivated Registered Nurse Case Manager to provide patient-centered care in the home health setting. The role involves assessing patients, coordinating care, educating patients and caregivers, supervising care staff, and maintaining clinical documentation. The position supports patients’ involvement and decision-making in collaborative plans of care, with the goal of promoting quality of life and holistic healing. Full-time and part-time opportunities are offered, with compensation of $38.00–$40.00 per hour.

Requirements

Graduate of an accredited nursing program.
Active Indiana RN license or a qualifying multistate license recognized by Indiana.
Minimum of 2 years of nursing experience.
Current CPR certification.
Strong communication, leadership, organizational, and clinical skills.
Commitment to providing exceptional patient-centered care.

Nice To Haves

Home health experience preferred.

Responsibilities

Conduct patient assessments and develop individualized plans of care.
Coordinate care with physicians, patients, families, and other healthcare professionals.
Educate patients and caregivers about care plans, treatments, and ongoing health needs.
Supervise and support LPNs, HHAs, and CNAs to promote quality patient care.
Review and update plans of care as needed.
Maintain accurate, timely, and complete clinical documentation.
Participate in case conferences and agency quality improvement initiatives.
Help ensure patients receive compassionate, safe, and effective care in their homes.
