About The Position

Hearth Hospice is seeking a full-time Hospice Registered Nurse Case Manager to provide end-of-life nursing care throughout the Polk and McMinn territory. The RN Case Manager delivers hands-on hospice care, coordinates services, and supports patients and families throughout their hospice journey as part of an interdisciplinary care team. The role emphasizes compassionate, clinically excellent care that promotes patient comfort, quality of life, dignity, and connection. The schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m., with shared call duties.

Requirements

• Hold a current, unencumbered Registered Nurse license.
• Have at least one year of experience as an RN in a medical-surgical or acute-care setting.
• Be proficient in typing and computer programs, including electronic medical records.
• Maintain current CPR certification if required by the state.
• Have a current driver's license, vehicle insurance, and access to dependable transportation.

Responsibilities

• Respond to clinical referral information and address inquiries and requests with care and expertise.
• Partner with the Clinical Director to assess client eligibility and suitability for hospice services.
• Coordinate care and provide direct nursing support to promote patient comfort and quality of life.
• Deliver nursing care using proven principles and techniques, including care that eases pain.
• Educate and advocate for patients and families, encouraging their participation in personalized care plans aligned with their goals.
• Share responsibility for call duties.
