About The Position

Amedisys is seeking a registered nurse case manager to provide home health care. The role involves assessing patients, collaborating with the care team to develop and implement plans of care, and helping patients heal at home. The position is PRN days. Compensation is $40.00 to $45.75 per hour and may vary by location, job-related knowledge, skills, and experience. Benefit eligibility may vary by position and shift status; participation in certain benefits requires enrollment in an Amedisys medical plan.

Requirements

• At least one year of clinical experience as a Registered Nurse; candidates with less than one year require approval by the VP Clinical.
• Current RN license specific to the state(s) assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• Candidates with less than six months of clinical RN experience must participate in the RN Intern program.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other disciplines as indicated by patients’ identified needs, or document the rationale for not doing so.
• Promote patient health and independence through teaching and appropriate rehabilitative measures, including helping patients learn self-care techniques.
• Supervise LPNs and HHAs.
• Complete documentation accurately and on time.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform assigned on-call responsibilities and provide on-call services to patients and families.
• Participate in clinical development and continuing education programs.
• Perform other duties as assigned.
