Hybrid RN Case Manager in Santa Fe/Espanola/Los Alamos
About The Position
United Energy Workers Healthcare is seeking a Registered Nurse Case Manager to assess patient needs, coordinate care, and support effective communication among everyone involved in patient care. The company provides compassionate, whole-person care to energy workers and their families under the Energy Employees Occupational Illness Compensation Program Act (EEOICPA). The role serves the Santa Fe, Espanola, and Los Alamos area and is described as hybrid. The Case Manager oversees nursing care, home health aide services, and applicable therapies after approval by the Department of Labor. The role also protects the confidentiality of client and office records under HIPAA guidelines and the Privacy Act, and facilitates communication and collaboration across the care team. Other duties may be assigned to support the team and patient care.
Requirements
- Hold a valid, active Registered Nurse license in good standing.
- Have at least one year of experience in a home health setting or related area, with strong assessment skills.
- Have excellent organizational and time management skills to manage multiple clients efficiently.
- Be proficient in computer and internet-based applications and office equipment.
- Have competent working knowledge of client-based equipment.
- Pass a criminal background check and sanction screening.
- Maintain a professional demeanor and appearance.
Responsibilities
- Oversee coordination of nursing care, home health aide services, and applicable therapies once approved by the Department of Labor.
- Maintain confidentiality of client and office records in accordance with HIPAA guidelines and the Privacy Act PHI.
- Facilitate effective communication and collaboration among the care team to provide cohesive, comprehensive care.
- Perform other duties as assigned to support the team and enhance patient care.
