The Nurse Case Manager (RNCM) will create, plan, and direct hospice care using the nursing process of assessment, intervention, implementation, and evaluation. This role involves collaboration with the Interdisciplinary Team and building relationships with patients and families, requiring a commitment to high standards of care and professional nursing clinical competency.
Requirements
- Current RN license in the State of Georgia
- Valid driver’s license, insurance, and reliable vehicle
- Utmost compassion and integrity
- Excellent time management skills
Nice To Haves
- 1–2 years of nursing experience
- 1–2 years of hospice experience
- Experience in skilled nursing, home health, acute care, med-surg, or ICU will be considered
Responsibilities
- Create, plan, and direct hospice care utilizing the nursing process of assessment, intervention, implementation, and evaluation.
- Collaborate with all members of the Interdisciplinary Team.
- Build meaningful relationships with patients and their families.
