About The Position

Amedisys is seeking a registered nurse case manager to provide home health care. The role focuses on assessing patients, coordinating and implementing individualized plans of care, supporting patient health and independence, and collaborating with the care team. The position is full-time, day shift, and includes assigned on-call responsibilities. The listed pay range is $65,000–$81,000, with compensation potentially varying by geographic market, job-related knowledge, skills, and experience.

Requirements

• At least one year of clinical experience as a Registered Nurse; candidates with less than one year require approval by the VP Clinical.
• Current RN license specific to the state or states where assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• Candidates with less than six months of clinical RN experience must participate in the RN Intern program.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement plans of care.
• Make referrals to other disciplines based on patients’ identified needs, or document the rationale for not making referrals.
• Promote patient health and independence through teaching and appropriate rehabilitative measures, including helping patients learn self-care techniques.
• Supervise LPNs and HHAs.
• Complete documentation accurately and on time.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform on-call responsibilities and provide on-call services to patients and families as assigned.
• Participate in clinical development and continuing education programs.
• Perform other duties as assigned.
