About The Position

Moments Hospice is hiring an RN Case Manager to serve as the clinical lead for hospice patients and their families. The role focuses on compassionate, patient- and family-centered care, including symptom management, care coordination, and guidance through end-of-life transitions. The position is field-based and works Monday through Friday, 8:30 AM to 5:00 PM. A fleet car program provides a company vehicle for business and personal use, with fuel and insurance covered at minimal cost to the employee.

Requirements

• Hold an active Registered Nurse (RN) license in the state where the employee works.
• Have proficiency with electronic medical records (EMR).
• Have strong organizational, communication, and critical-thinking skills.
• Be able to work independently while maintaining high standards of care.
• Have a compassionate, patient- and family-centered approach.
• Have a valid driver's license and a reliable vehicle with current insurance.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plans of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers about disease progression, symptom management, medications, and what to expect at end of life.
• Coordinate care across the caseload and serve as the primary liaison between patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.
