RN Case Manager
About The Position
Independence Home Health LLC is hiring an RN Case Manager to serve patients in Bloomington, Indiana, and surrounding areas. The role involves in-home patient visits and emphasizes compassionate, high-quality care that supports patients’ independence and participation in decisions about their healthcare. The nurse collaborates with patients, families, physicians, and the care team to develop and carry out individualized plans of care while respecting patients’ preferences and freedom. The position includes scheduled home visits. The nurse reports to the office Monday through Friday at 8:00 AM to pick up a company vehicle before completing visits. Compensation is $38.00–$41.00 per hour.
Requirements
- Current, unrestricted Registered Nurse (RN) license in Indiana.
- Strong communication, organization, and time-management skills.
- Compassionate, dependable, and patient-focused approach to care.
- Ability to work independently while collaborating with a multidisciplinary team.
- Valid driver’s license and ability to meet company vehicle insurance requirements.
Nice To Haves
- Home health experience preferred.
- Case management experience preferred.
- Experienced RNs are encouraged to apply; candidates with strong nursing skills and a desire to grow in home health are welcome.
Responsibilities
- Complete patient assessments and develop individualized plans of care.
- Coordinate and oversee patient care to ensure quality and continuity of services.
- Monitor patient progress and communicate changes in condition to appropriate healthcare professionals.
- Conduct scheduled home visits throughout Bloomington and surrounding areas.
- Collaborate with physicians, patients, families, and the care team.
- Maintain accurate, timely clinical documentation in accordance with agency policies and applicable regulations.
- Educate patients and caregivers about care plans, medications, and health-related needs.
- Help ensure services meet established clinical standards and home health requirements.
