About The Position
A leading home health organization in Salem, Oregon is seeking an RN Case Manager to join its growing clinical team! This is a full-time, permanent, direct-hire position with a Monday-Friday, 8:00 AM - 5:00 PM schedule. The role offers guaranteed weekly pay, mileage reimbursement, and relocation assistance. A comprehensive benefits package including medical, dental, vision, retirement benefits, and paid time off is provided.
Requirements
- Current Oregon Registered Nurse (RN) license required
- Minimum of one year of recent acute care experience required
- Current BLS/CPR certification required
- Valid driver's license and auto insurance required

Responsibilities
- Complete patient assessments and develop individualized plans of care
- Monitor patient progress and revise care plans as needed
- Administer medications and treatments according to physician orders
- Educate patients and families regarding health conditions and care plans
- Coordinate care with physicians and interdisciplinary healthcare teams
