About The Position

Heart to Heart Hospice provides compassionate care to patients with life-limiting illnesses and their loved ones. The RN Case Manager assesses how a terminal disease affects patients’ physical, psychosocial, and environmental needs and activities of daily living; implements and revises individualized plans of care; and coordinates care with patients, caregivers, physicians, and other healthcare professionals. The role participates in interdisciplinary care planning, provides assigned on-call service, completes documentation promptly, and helps ensure compliance with applicable laws, regulations, and agency policies. A $5,000 sign-on bonus is offered.

Requirements

Graduate of an accredited diploma, associate, or baccalaureate school of nursing.
Current state Registered Nurse license.
Current driver’s license, reliable transportation, and ongoing valid and current auto liability insurance.

Nice To Haves

Two years of RN experience, preferably in a healthcare or clinical setting.

Responsibilities

Conduct initial and ongoing comprehensive assessments of the impact of terminal illness on patients’ physical, psychosocial, and environmental needs and activities of daily living, including grief risk and cultural, spiritual, verbal, and non-verbal needs.
Implement individualized plans of care and recommend revisions as necessary.
Collaborate on care with patients, caregivers, physicians, and healthcare professionals.
Attend Inter-Disciplinary Group meetings and coordinate and participate in patient care planning, prioritization of needs, and coordination of care.
Perform assigned on-call responsibilities and provide on-call service to patients and families.
Submit all documentation in a timely manner.
Perform other duties as required.
Ensure compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
