RN Case Manager ($10,000 Sign On Bonus)
About The Position
Community Care is hiring a full-time RN Case Manager for its Family Care Program, working out of the Waukesha location. The position supports the organization’s mission to help elderly individuals and adults with physical and intellectual disabilities live as independently as possible in the community. The RN Case Manager collaborates with a team and carries out community case management, including assessments, home visits, care planning, and follow-up on services. Community Care is a local nonprofit with more than 40 years of experience helping older adults and adults with disabilities live independently in the community. It coordinates and delivers supportive services to more than 13,000 Wisconsin residents, with local offices serving 23 counties. The organization works with members and community partners to develop care plans addressing health and social needs and to provide compassionate, personalized care. The role is scheduled Monday through Friday, 8:00 AM to 4:30 PM. A $10,000 sign-on bonus is offered.
Requirements
- Associate degree in Nursing required.
- Wisconsin Licensed Registered Nurse.
- Eligibility for long-term care functional screener certification required.
- Wisconsin driver’s license.
- Two years of recent nursing experience with older adults or people with physical or intellectual disabilities.
Nice To Haves
- Bachelor’s degree preferred.
- Two years of work in community nursing preferred.
Responsibilities
- Complete discipline-specific initial and ongoing assessments.
- Conduct home visits and home assessments as needed.
- Consult with families and other supports for additional information as needed.
- Develop and maintain care plans, including members’ long-term care and personal outcomes, progress toward previously identified outcomes, strengths, risks, and natural and formal supports.
- Follow up on services to ensure their quality, coordination, and effectiveness.
