About The Position

Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The RN Case Manager coordinates individualized hospice care by assessing patients, partnering with the interdisciplinary care team, and developing plans that support each patient's needs. Patient coverage is in the Bourne and Falmouth areas; orientation takes place at the care center in Hyannis. The role includes assigned on-call responsibilities and services to patients and families.

Requirements

• Current RN license specific to the state(s) where assigned to work.
• At least 1 year of nursing experience.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Assess patients and collaborate with the care team to develop and implement plans of care.
• Make referrals to other hospice care disciplines as needed, or document the rationale for not doing so.
• Develop, prepare, and maintain individualized patient care progress records accurately, promptly, and according to care center policies.
• Record pain and symptom management changes and outcomes as appropriate.
• Teach patients, families, and caregivers appropriate patient care techniques.
• Communicate regularly with the clinical manager and care team.
• Perform assigned on-call responsibilities and provide on-call services to patients and families.
• Perform other duties as assigned.
