About The Position

Heart’n Soul Hospice is seeking an RN Case Manager to provide compassionate hospice care to patients and families facing life-limiting illness. The role includes assessing patients’ physical, psychosocial, and environmental needs; implementing and revising individualized plans of care; and coordinating care with patients, caregivers, physicians, healthcare professionals, and the Inter-Disciplinary Group. The RN Case Manager also performs assigned on-call duties, submits documentation promptly, and ensures compliance with applicable laws, regulations, and agency policies. A $10,000 sign-on bonus is offered.

Requirements

Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
Current state Registered Nurse license.
Current driver’s license, reliable transportation, and ongoing valid and current auto liability insurance.

Nice To Haves

Two years of RN experience in a healthcare or clinical setting is preferred.

Responsibilities

Conduct initial and ongoing comprehensive assessments of how terminal disease affects patients’ physical, psychosocial, and environmental needs and activities of daily living, including grief risk and cultural, spiritual, verbal, and non-verbal needs.
Implement individualized plans of care and recommend revisions as needed.
Coordinate care among patients and caregivers, physicians, and other healthcare professionals.
Attend Inter-Disciplinary Group meetings; coordinate and participate in patient care planning, prioritize needs, and collaborate with the group to promote coordinated care.
Perform assigned on-call responsibilities and provide on-call services to patients and families.
Submit all documentation in a timely manner.
Perform other duties as required.
Ensure compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
