About The Position

Amedisys is hiring a full-time registered nurse case manager to provide day visits in Bedford and surrounding communities. The role supports patients receiving care at home, coordinating with the care team to help patients heal, maintain independence, and learn self-care. The position includes a $10,000 sign-on bonus and an advertised salary range of $91,000 to $110,000. Benefit eligibility may vary by position and shift status; participation in some benefits requires enrollment in an Amedisys medical plan.

Requirements

• At least one year of clinical experience as a Registered Nurse; candidates with less than one year must be approved by the VP Clinical.
• Current RN license specific to the state or states assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• Candidates with less than six months of clinical RN experience must participate in the RN Intern program.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Refer patients to other disciplines as indicated by identified needs, or document the rationale for not doing so.
• Promote patient health and independence through teaching and appropriate rehabilitative measures, including self-care education.
• Supervise LPNs and HHAs.
• Complete documentation accurately and on time.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform assigned on-call responsibilities and provide on-call services to patients and families.
• Participate in clinical development and continuing education programs.
• Perform other duties as assigned.
