About The Position

Community Care is hiring a Registered Nurse (RN) for its Family Care Program in Milwaukee, Wisconsin. The role supports elderly individuals and adults with physical and intellectual disabilities in continuing to live as independently as possible in the community. The RN manages a caseload and provides community nursing case management, clinical assessments, care coordination, member education, and monitoring of services. The position involves regular local travel using a personal vehicle.

This is a Monday through Friday day-shift position, 8:00 a.m. to 4:30 p.m. The posting offers a $10,000 sign-on bonus and describes an emphasis on team collaboration and work-life balance.

Requirements

• Associate degree in Nursing.
• Wisconsin Registered Nurse (RN) license.
• At least 3 years of recent nursing experience with older adults or people with physical or intellectual disabilities.
• Eligibility for current long-term care functional screener certification.
• Valid driver's license and proof of required vehicle insurance.

Nice To Haves

• Bachelor's degree in Human Services or a closely related field.
• Current long-term care functional screener certification.
• 3 years of community nursing work.

Responsibilities
