About The Position

Brookhaven Hospice is seeking a full-time Hospice Registered Nurse Case Manager to provide end-of-life care throughout the Southern Brookhaven territory. The role combines hands-on hospice nursing, care coordination, and support for patients and families throughout the hospice journey. The RN Case Manager works as part of an interdisciplinary care team to provide compassionate, clinically excellent care and help ensure patients’ comfort and quality of life.

The schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m., with shared call duties. The listed average pay range is $95,000–$105,000 per year; final compensation depends on relevant experience, qualifications, and position responsibilities and is confirmed at the end of the interview process. A $15,000 sign-on bonus is offered.

Requirements

• Current, unencumbered Registered Nurse license.
• At least one year of RN experience in a medical-surgical or acute-care setting.
• Computer proficiency, including typing and use of various programs, with an EMR background.
• Current CPR certification if required by the state.
• Current driver’s license, vehicle insurance, and access to dependable transportation.

Responsibilities

• Respond to clinical referral information and address inquiries and requests.
• Work with the Clinical Director to assess clients’ eligibility and suitability for hospice services.
• Provide direct nursing care and support patients’ comfort and quality of life.
• Deliver nursing care using established principles and techniques to help ease pain and provide comfort.
• Educate and advocate for patients and families, encouraging their participation in personalized care plans.
• Share responsibility for call duties.
