About The Position

Continuum Hospice is seeking a full-time Hospice Registered Nurse Case Manager to provide end-of-life care throughout the community. The role is based on a Monday–Friday schedule, 8:00 a.m.–4:30 p.m., and serves South Boston, Braintree, Dorchester, Quincy, Randolph, Milton, and Weymouth. The RN Case Manager delivers hands-on hospice nursing care, coordinates services, and supports patients and families throughout their hospice journey as part of an interdisciplinary care team. The position includes shared call duties. A $15,000 sign-on bonus is offered. The listed average pay range is $95,000–$105,000 per year; final compensation depends on relevant experience, qualifications, and role responsibilities.

Requirements

Hold a current, unencumbered Registered Nurse license.
Have at least one year of RN experience in a medical-surgical or acute-care setting.
Be proficient in typing and computer programs, including electronic medical records.
Maintain current CPR certification if required by the state.
Have a current driver's license, vehicle insurance, and access to dependable transportation.

Nice To Haves

Hospice experience is a plus.

Responsibilities

Respond to clinical referral information and address inquiries and requests.
Partner with the Clinical Director to assess client eligibility and suitability for hospice services.
Provide direct nursing care and support patients’ comfort and quality of life.
Deliver nursing care using established principles and techniques to help ease pain and provide comfort.
Educate and advocate for patients and families, encouraging participation in personalized care plans aligned with their goals.
Share responsibility for call duties.
