About The Position

This is a great opportunity for a nurse who loves nursing and wants to diversify their experience. As a home health nurse, you will work a 4/10 schedule allowing for more work-life balance. The position is located in beautiful northwest Arizona, in the city of Kingman, which has a mild climate with stunning Arizona sunsets, is near beautiful mountain ranges and lakes, and is an outdoor enthusiasts' paradise with abundant sunshine. The Registered Nurse (RN) coordinates the total plan of care for the patient in the home care setting, assessing, planning, implementing, and evaluating.

Requirements

Graduate of a nursing school program
AZ RN License, or compact state RN License
Level One Fingerprint Clearance Card, or application, required within 20 days of hire
AZ Driver's License
BLS Certification from American Heart Association or American Red Cross
1-2 years acute care nursing or home health experience

Responsibilities

The Home Health RN coordinates the implementation of a nursing plan of care for each assigned patient.
Works under the direction of the physician and the Arizona State Nurse Practice Act to provide nursing care to patients in compliance with nursing standards and regulations.
Utilizes the nursing process to make continuing assessments of the patient’s total health needs; implements nursing interventions based on assessment data; develops and implements the nursing plan of care and evaluates the patient’s response.
Collaborates with healthcare providers, nursing staff, other patient caregivers, case managers, and health information team members to improve the quality and completeness of documentation of care.
Adheres to behavioral expectations and policies; maintains patient and staff confidentiality; supports a healthy work environment through skilled communication, true collaboration, effective decision making, and meaningful recognition.
