About The Position

Amedisys is hiring a Registered Nurse Case Manager for a full-time Baylor weekend role in home health and hospice care. The nurse helps patients heal at home by assessing their needs, coordinating and implementing plans of care, supporting patient health and independence, and communicating progress to the clinical manager and care team. The role also supervises LPNs and home health aides, provides staff education, and includes assigned on-call responsibilities.

The advertised pay range is $73,000–$90,000. A $15,000 sign-on bonus is stated in the job title. Compensation may vary by location and by job-related knowledge, skills, and experience. Requirements

• At least one year of clinical experience as a Registered Nurse; candidates with less than one year require approval by the VP Clinical.
• Current RN license specific to the state or states assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• Candidates with less than six months of RN clinical experience must participate in the RN Intern program.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Refer patients to other disciplines as indicated by identified needs, or document the rationale for not doing so.
• Promote patient health and independence through teaching and appropriate rehabilitative measures, including teaching self-care techniques.
• Supervise LPNs and home health aides.
• Complete documentation timely and accurately.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform assigned on-call responsibilities and provide on-call services to patients and families.
• Participate in clinical development and continuing education programs.
• Perform other duties as assigned.
