RN Case Manager
About The Position
Anew Hospice is hiring a full-time Registered Nurse Case Manager serving the Lafayette, Indiana area. Anew Hospice is a local, family-owned company operating exclusively in Indiana. The RN delivers patient care through assessment, planning, implementation, and evaluation; plans, organizes, and directs hospice care services; and draws on community resources to coordinate services for individuals and families in their homes and communities. The role emphasizes nursing experience and community health education or experience. The position follows a Monday–Friday, daytime schedule, with nights free, and includes mileage reimbursement.
Requirements
- Valid Indiana Registered Nurse (RN) license.
Nice To Haves
- Previous hospice or long-term care experience is preferred.
Responsibilities
- Deliver patient care through the nursing process of assessment, planning, implementation, and evaluation.
- Plan, organize, and direct hospice care services.
- Use community resources to plan and direct services that meet the needs of individuals and families in their homes and communities.
