RN Case Manager
About The Position
St. Croix Hospice provides compassionate end-of-life care, helping patients and families pursue the highest possible quality of life with dignity, comfort, and respect. The Registered Nurse Case Manager provides one-to-one, whole-person care to patients and families in their homes. Working with an integrated care team, the nurse coordinates individualized care plans, supports families through complex decisions, and provides comfort with professionalism, empathy, and respect. The position is based in Stevens Point, Wisconsin, with 100% travel and an 8:00 a.m. to 5:00 p.m. schedule, plus on-call expectations.
Requirements
- Associate, bachelor's, or master's degree from an accredited nursing program (RN).
- At least 1 year of recent acute care experience.
- Reliable transportation, a valid driver’s license, and proof of auto insurance.

- Hospice or home health experience.
Responsibilities
- Complete initial and ongoing comprehensive assessments to determine hospice needs.
- Develop individualized care plans with patients and families and update them as needs evolve.
- Administer medications and treatments as prescribed.
- Educate patients and caregivers on care techniques, medications, diet, and symptom management.
- Rotate on-call duties and ensure seamless care transitions.
- Coordinate individualized care plans in partnership with an integrated care team.
- Support families through complex decisions and provide compassionate care in patients’ homes.
