RN Case Manager - PRN

About The Position

Empath Health is seeking a PRN RN Case Manager to serve Manatee and Northern Sarasota Counties. Under the direction of a licensed physician, the nurse manages discomfort and provides symptom relief using specialized nursing skills related to palliative and end-of-life care. The role includes assessing how a terminal diagnosis affects the patient’s physical, functional, psychosocial, and environmental needs and activities of daily living. In collaboration with the Interdisciplinary Group, the RN develops, implements, and updates individualized plans of care, initiates preventive and rehabilitative procedures and service referrals, and requests complimentary services as needed. The RN also educates patients or residents, families, and caregivers about disease progression, palliative interventions, caregiving, the dying process, and safety practices.

Requirements

Graduate of an approved nursing program.
Current unrestricted Florida RN license.
Valid and current CPR certification.
Minimum of one year of experience providing direct patient care in an acute-care hospital environment.

Nice To Haves

Previous experience as a Hospice RN preferred.
CHPN certification strongly encouraged within three years of employment.
BSN or MSN preferred.

Responsibilities

Manage discomfort and provide symptom relief using specialized palliative and end-of-life nursing skills under the direction of a licensed physician.
Conduct initial and ongoing assessments of the effects of the terminal diagnosis on patients’ physical, functional, psychosocial, and environmental needs and activities of daily living.
Collaborate with the Interdisciplinary Group to develop, implement, and update individualized plans of care.
Initiate appropriate preventive and rehabilitative procedures and referrals to other services; request complimentary services as needed.
Educate patients or residents, families, and caregivers about disease progression and decline, palliative interventions, caregiving, the dying process, and safety practices.
