About The Position

Holisticare Hospice is seeking a Per Diem Registered Nurse Case Manager to serve patients and families in and around Atlantic and Cape May Counties. The role is scheduled for evenings about three times a week. The RN Case Manager plans, organizes, and directs hospice care through assessment, planning, interventions, implementation, and evaluation, while maintaining professional nursing standards and clinical competency. The position involves interacting with patients, caregivers, families, and interdisciplinary team members to provide compassionate, patient-centered end-of-life care.

Requirements

• Current, unencumbered Registered Nurse license.
• At least one year of RN experience in a medical-surgical or acute care setting.
• Computer proficiency, including typing and use of various programs, with a background in electronic medical records (EMR).
• Current CPR certification if required by the state.

Nice To Haves

• Spanish-speaking ability preferred.
• Hospice experience is a plus.

Responsibilities

• Respond compassionately and expertly to clinical referral information, inquiries, and requests.
• Work closely with the Clinical Director to assess clients’ eligibility and suitability for hospice services.
• Provide direct nursing care and support to promote patients’ comfort and quality of life.
• Deliver nursing care using established principles and techniques, including care to ease pain.
• Educate and advocate for patients and families, supporting their participation in personalized care plans aligned with their goals.
• Share responsibility for call duties.
