About The Position
Baptist Health is seeking a dedicated and compassionate Registered Nurse to join its Case Management team. This PRN day-shift role supports patients throughout their hospital stay by ensuring coordinated, high-quality care. The position is located at Beaches - Main Building in Jacksonville Beach, Florida.
Requirements
- At least 1 year of case management experience.
- Associate of Science in Nursing required.
- Basic Life Support (BLS) certification required.
- Florida Licensed Registered Nurse required.
- Strong communication and care-coordination skills.
Nice To Haves
- Bachelor of Science in Nursing preferred.
Responsibilities
- Provide comprehensive care coordination for hospitalized patients.
- Conduct high-risk screenings and assessments to identify patient needs.
- Arrange and document post-acute care services to ensure smooth transitions of care.
- Collaborate with a multidisciplinary healthcare team to support optimal patient outcomes.
