About The Position
Baptist Health is seeking a Registered Nurse Case Manager for the In-Patient Case Management Department at the Baptist Medical South campus. This is a PRN weekend opportunity. The role coordinates care for hospitalized patients, assesses high-risk patients, arranges post-acute care services with appropriate documentation, and collaborates with multidisciplinary team members. The primary location is the South Main Building at 14550 Old St Augustine Road, Jacksonville, Florida 32258.
Requirements
- At least 1 year of Case Management experience.
- Associate's Degree in Nursing (ADN).
- Basic Life Support (BLS) certification.
- Driver's license (DMV).
- Active Florida Registered Nurse (RN) license.
Responsibilities
- Coordinate care for hospitalized patients.
- Conduct high-risk screenings and assessments.
- Coordinate post-acute care services and complete proper documentation.
- Collaborate with multidisciplinary team members.
