About The Position
Community Care, Inc. is seeking a compassionate and dedicated Registered Nurse to provide home health care to clients in the Westmoreland and Fayette County area. The role involves providing personalized care in patients’ homes and building meaningful connections with clients. The schedule is Saturday and Sunday, with varying hours, on the day shift. The position is based in New Stanton, Pennsylvania.
Requirements
- Current Registered Nurse (RN) license in the state of Pennsylvania.
- Graduate of an accredited nursing program.
- Minimum of 1 year of clinical nursing experience.
- CPR certification.
- Reliable transportation and a valid driver’s license.
- A caring disposition, strong independence, and a passion for pediatric care.
Nice To Haves
- Home Health experience preferred.
Responsibilities
- Conduct skilled nursing visits in patients’ homes, including assessments, reassessments, medication administration, IV therapy, and treatments.
- Develop and implement individualized care plans.
- Educate patients and caregivers on managing conditions and medications.
- Collaborate with a multidisciplinary team to ensure continuity of care.
- Document visits and patient progress accurately and promptly.
