About The Position

Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Registered Nurse (RN) Case Manager for Element Care PACE in Lynn, Massachusetts. This full-time position is part of the PACE Program and focuses on community-based case management. The role involves coordinating comprehensive care for older adults enrolled in the Program of All-Inclusive Care for the Elderly (PACE). The goal is to develop, implement, and monitor individualized care plans that promote independence and reduce unnecessary hospitalizations. The RN Case Manager will conduct participant assessments, coordinate services across medical, behavioral health, therapy, and community resources, and serve as the primary nursing case manager while collaborating with an interdisciplinary team. Maintaining continuity of care through ongoing participant follow-up, education, and care coordination, as well as completing timely documentation utilizing the electronic medical record system, are key aspects of the position.

Requirements

  • Active Registered Nurse (RN) license in Massachusetts.
  • Minimum two years of Registered Nurse experience.
  • Strong assessment, care coordination, and communication skills.
  • Ability to work independently while collaborating within an interdisciplinary team.
  • Valid driver's license and reliable transportation for community visits.

Nice To Haves

  • Home health, case management, community health, or geriatric nursing experience preferred.
  • Electronic medical record experience preferred.

Responsibilities
