About The Position
Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Bilingual Registered Nurse (RN) for Element Care PACE in Lynn, Massachusetts. This role involves providing comprehensive nursing care for older adults enrolled in the Program of All-Inclusive Care for the Elderly (PACE). The nurse will perform initial and ongoing nursing assessments in both clinic and community settings, develop and implement individualized care plans to promote independence and reduce hospitalizations, and collaborate with a multidisciplinary care team. Responsibilities also include educating participants and caregivers on disease management, medications, and preventative health, and maintaining accurate clinical documentation in the electronic medical record.
Requirements
- Active Registered Nurse (RN) license in Massachusetts.
- Bilingual in English and Spanish.
- Minimum two years of recent Registered Nurse experience.
- Ability to provide nursing care in both clinic and home/community settings.
- Strong assessment, communication, and documentation skills.
- Valid driver's license and reliable transportation.
Nice To Haves
- Experience in primary care, geriatrics, home health, or community health is preferred.
Responsibilities
- Provide comprehensive nursing care for older adults enrolled in the Program of All-Inclusive Care for the Elderly (PACE).

- Collaborate with physicians, therapists, social workers, and the interdisciplinary care team to coordinate participant care.

- Maintain accurate clinical documentation within the electronic medical record.
