About The Position

Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Physical Therapist (PT) for Element Care PACE in Brighton, Massachusetts. This role involves providing comprehensive physical therapy evaluations, treatment planning, therapeutic interventions, and home safety assessments for older adults enrolled in the PACE program. Care will be delivered in both the Adult Day Health Center and participants' homes throughout the service area. The position focuses on developing individualized plans of care to maintain independence and prevent functional decline, with opportunities to supervise Physical Therapist Assistants (PTAs) and rehabilitation aides. The Physical Therapist will collaborate with an interdisciplinary PACE team while maintaining autonomy in clinical decision-making.

Requirements

• Active Massachusetts Physical Therapist License REQUIRED
• Minimum One Year of Physical Therapy Experience with Older Adults REQUIRED
• Experience Completing Evaluations, Treatment Plans, and Home Safety Assessments REQUIRED
• Ability to Supervise Physical Therapist Assistants (PTAs) and Rehabilitation Aides
• Strong Communication and Interdisciplinary Team Collaboration Skills

Nice To Haves

• Experience Providing Care in Clinical and Home-Based Settings Preferred
