About The Position

The RN Case Manager provides quality case management and qualified professional support and services to inpatient social workers as assigned. This role works with local agency and service providers to facilitate sending and receiving collateral information, treatment referrals, and ongoing communications to support patient’s successful inpatient treatment and discharge. Key responsibilities include reviewing clinical documentation from referral sources to ensure direct admissions are appropriate for level of care, coordinating bed availability and placement with unit nurses and nursing leadership, meeting with patients to provide education on initial treatment planning and safety goals, prioritizing bed placement based on patient acuity, safety considerations, and census needs, participating in quality improvement initiatives by reviewing nursing assessment documentation, patient consents, and ensuring insurance pre-certifications are completed, and providing a thorough report of patient presentation and necessary safety precautions to unit nurse to support a safe transition to the unit.

Requirements

  • Registered Nurse degree and license
  • A graduate of a college, or university with a Masters degree in a human services field and has one year of full-time, post graduate accumulated mental health experience with the population served and a substance abuse professional shall have one year of full-time post-graduate accumulated supervised experience in alcoholism and drug abuse counseling
