About The Position

Amedisys is hiring a registered nurse case manager to provide home health care, helping patients heal at home. The role includes assessing patients, working with the care team to plan and implement care, supporting patient health and independence, and coordinating with other care disciplines. The position is PRN. Pay is $38.00–$45.00, with compensation varying by geographic market and job-related knowledge, skills, and experience.

Requirements

• At least one year of clinical experience as a Registered Nurse; candidates with less than one year must be approved by the VP Clinical.
• Current RN license specific to the state or states where assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• Candidates with less than six months of RN clinical experience must participate in the RN Intern program.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other disciplines based on patients’ identified needs, or document the rationale for not making referrals.
• Promote patient health and independence through teaching and appropriate rehabilitative measures, helping patients learn self-care techniques.
• Supervise LPNs and HHAs.
• Complete documentation accurately and on time.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform on-call responsibilities and provide on-call services to patients and families as assigned.
• Participate in clinical development and continuing education programs.
• Perform other duties as assigned.
