About The Position

The Hospital Case Manager RN coordinates high-quality care across the health care continuum. The role collaborates with physicians and advanced practice providers, peers, community colleagues, and others to coordinate care, identify opportunities to improve patient care and services, improve population health, and support collaborative, patient-centered care and community engagement. The position is based at MultiCare Covington Medical Center, a 58-bed facility serving southeast King County.

Requirements

• Bachelor's degree in Nursing, or obtain it within five years from the initial date of hire. RN employees with 15 or more years of service and internal candidates hired before January 1, 2021 may be considered in satisfaction of this requirement in lieu of an education agreement.
• Current Registered Nurse license in Washington State or Multistate License endorsement (MLS).
• Four years of recent hospital case management experience.
• Five years of recent clinical hospital or ambulatory experience.

Nice To Haves

• Advanced degree in Nursing (MSN, PhD, DNP, or ARNP).
• Healthcare financial and reimbursement knowledge.
• Leadership and emergency services experience.

Responsibilities

• Ensure patients and families experience medically appropriate transitions of care as an integral part of care delivery.
• Advocate for patients and families, including transitions to a lower level of care when inpatient care is no longer needed.
• Partner with and support physicians and advanced practice providers in patient care activities and care transitions.
• Use critical thinking in all interactions.
• Be accountable for transitions of care and care across the continuum.
