About The Position

Beacon Hospice of San Antonio is seeking a full-time Registered Nurse Case Manager for the San Antonio area. This is an outpatient position requiring an independent, experienced RN. As an integral member of the interdisciplinary care team, the RN Case Manager admits patients and coordinates appropriate care for each patient and family. Main duties include admissions, assessments, discharges, and pronouncements. The schedule is 8:00 a.m. to 5:00 p.m., Monday through Friday. On-call and weekend rotation may be required.

Requirements

• Licensed RN in the State of Texas.
• Effective written and verbal communication skills.
• Ability to travel and reliable transportation.
• Clean background, drug screen, and driving record.

Nice To Haves

• At least 1 year of experience as an RN.
• Experience with hospice or home health.

Responsibilities

• Admit patients.
• Coordinate appropriate care for each patient and family as part of the interdisciplinary care team.
• Complete patient assessments.
• Complete discharges.
• Perform pronouncements.
