About The Position

This role involves creating a plan for care across the continuum, integrating patient/family preferences and values. The RN Case Manager will monitor patient care through assessments, evaluations, and/or patient records. They will advocate for resources and the removal of barriers, maintaining ongoing dialogue with supervisors and other health providers to ensure effective implementation of the health plan. Additionally, the role serves as a resource for adequate medical record documentation, appropriateness of services related to diagnoses, and treatment options for post-discharge care.

Requirements

Licensed Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required.
Licensed as a Registered Nurse in Florida required.
Credentialed from State Board of Nursing or current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date required.
Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

Nice To Haves

BLS Provider preferred. American Heart Association or American Red Cross accepted.
