This is a regional leadership position within our agency, requiring travel as needed to all counties throughout Southwest & Mid-Central Ohio where clients and caregivers reside. This role works closely with and reports directly to our SW OH Regional Program Administrator (local) and/or Director of Clinical Administration & Operations (corporate). For those pursuing career growth: a defined path for advancement to Assistant Program Administrator, Program Administrator, Clinical Administrator, Director of Nursing (DON or Asst. DON), Business Administrator, QA / Compliance leadership, and other positions will be supported. Work consists of: case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services, as well as new hire orientations, in-service and supervisory reviews, and input to influence our training and education, care practices and outcomes, and the success of our clients, employees and business overall. Requires back-up and occasional Skilled Nursing visits and services including med sets, dressing changes, etc. At least 1 year of experience is required in: Case Management, Case Admissions and Discharges, wound care, PICC/PORT/IV Care, Transitional Care, lab draws, OASIS and Non-OASIS assessments, and excellent documentation and field staff leadership. At least 1 year of paid experience is required at/with a home health agency or in-home care provider (or equivalent); 2-5 years' experience is preferred. Willingness to travel throughout this region is required (up to approx. 2-hour daily radius). Primary / Centralized service areas & required travel: Dayton-Middletown-Springfield Metro, Washington Court House-Wilmington Metro. Secondary service areas & required travel (aka: less frequent but occasional) span all Southwest Ohio and South-Central Ohio communities, including those in and near: Dayton, Middletown, Springfield, Piqua, Urbana, Cincinnati & Cincy Metro areas, Washington Court House, Salem Township, Wilmington, Circleville, Columbus & immediate Columbus Metro area. Qualified Applicants may reside in or near any part of the service areas so long as travel requirements are fulfilled. NO overnight travel required. This is a Full-time, Salaried position requiring weekday availability (Monday - Friday) between 8am - 5pm, with occasional/rare evening availability (scheduled in advance). No On-Call Shifts / No Weekends required. Weekend services are optional when available but are not required. Salaried employees are paid via direct deposit 2x monthly. With core skills in managed care, patient assessments and compliance, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care. You will submit updated documentation of the patient's condition and ongoing care needs timely and within compliance of all state, federal, and agency guidelines. You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client + his/her family.
