The RN, Case Manager is responsible for assessing and identifying patient/family needs, utilizing the nursing process, coordinating the Plan of Care with the Interdisciplinary Team (IDT), and providing clinical, palliative and supportive care to the patient/family unit in order to keep the participant in their home environment as long as possible. This role involves providing reassurance on the phone, assisting with solutions, recognizing the need for in-person visits, and coordinating those visits when necessary. The position also involves utilizing appropriate support/expert resources to resolve complex situations, documenting patient/family contact information, and completing initial and semi-annual assessments. The RN will explain services, address questions and concerns, present services empathetically, and provide information to physicians and other IDT members. They will initiate skilled nursing interventions, obtain physician orders, and complete documentation per company policy. Additionally, the RN acts as the Company representative at assigned facilities, facilitates referrals, and communicates with referring entities. They provide timely clinical communication using SBAR, discuss potential needs with after-hours staff, and develop strong relationships with case managers and physicians at facilities. The role also includes supervising patient care provided by Community Health Workers and Home Health Aides as requested. During emergencies like hurricanes, the RN may be required to report to a designated company location to ensure continuity of services, potentially including reporting before the scheduled date/time and staying overnight.
