About The Position

The Registered Nurse Assessment Coordinator (RNAC) coordinates the MDS and Care Planning process to ensure timely and accurate submission with the Interdisciplinary team. This is a PRN remote position supporting multiple locations, limited to Erie, Pennsylvania, and may offer up to 40 hours per week. The role requires a Registered Nurse license in both Pennsylvania and Ohio and at least three years of experience as an MDS Nurse. The coordinator works with nursing staff, the Case Manager, and the Interdisciplinary team to complete and submit assessments and care plans on schedule. The position also participates in QAPI and accesses, prints, and evaluates Quality Indicators.

Requirements

Graduate from an accredited school of Nursing.
Hold a current Registered Nurse license from the State Board of Nursing in both Pennsylvania and Ohio.
Have at least three years of experience as an MDS Nurse.
CPR certification is recommended.
Demonstrate compassion, teamwork, social skills, trustworthiness, and a service-minded approach.
Be self-motivated, dependable, ethical, and ready to provide outstanding care and customer service to residents.

Responsibilities

Assist with nursing staff education in documentation, MDS, CAAs, and the Care Plan process.
Accurately complete MDS assessments within established time frames for new admissions, quarterly and annual assessments, and significant changes.
Precheck and electronically submit MDS assessments to the State agency, review validation reports, and make necessary changes.
Coordinate scheduling and timely completion of MDS assessments and Care Plans with the Case Manager and Interdisciplinary team.
Participate in the QAPI process.
Electronically access, print, and evaluate Quality Indicators.
