About The Position

Tidewell Hospice, a member of Empath Health, provides compassionate hospice care to families across southwest Florida. The RN Triage Clinical Coordinator provides after-hours phone support and clinical guidance to clients, community partners, and colleagues. This role coordinates after-hours information, patient care, visits, and services across teams and community partners, while monitoring workflow and helping direct-care staff respond efficiently to changing needs. The coordinator uses hospice nursing assessment, observation, intervention, and education skills to promote comfort, quality of life, and symptom management. The position also performs telehealth visits and supports continuity of care through real-time documentation, timely reporting, collaboration, and communication. This is a PRN remote role for local candidates who can work flexible hours as needed. Typical coverage is from 5:00 PM to 8:00 AM, with availability required for short-notice coverage of call-offs and other staffing needs.

Requirements

Hold a clear and unrestricted Florida or multistate RN license.
Have at least one year of recent experience, within the last three years, in acute care, post-acute care, inpatient, or hospice settings.
Be local and available to work flexible hours as needed, including short-notice coverage for call-offs and other staffing needs.

Nice To Haves

BSN or MSN preferred.
CHPN preferred or encouraged within two years of employment.

Responsibilities

Provide tiered after-hours phone support, specialized knowledge, clinical guidance, and direction to clients, community partners, and colleagues.
Coordinate after-hours information, patient care and visits, and services among teams, departments, and community partners.
Monitor workflow and assist direct-care staff in responding rapidly, efficiently, and accurately to changing needs.
Use hospice-specific nursing assessment, observation, intervention, and education skills to maximize comfort, enhance quality of life, and manage symptoms.
Perform telehealth visits to provide immediate virtual nursing assistance.
Promote continuity of care through real-time documentation, timely reporting, collaboration, and communication.
