About The Position

The Residential Case Manager provides individualized case management and advocacy services to adults living in a residential facility for individuals with mental illness. This role ensures that each resident receives the supports necessary to maintain stability, pursue recovery goals, and work toward greater independence. The Case Manager works closely with clinical and residential staff to coordinate services, monitor progress, and connect residents with community-based resources.

Requirements

  • Bachelor's degree in social work, psychology, or a related field.
  • Minimum of 2 years of experience in case management or direct service provision to adults with mental illness.
  • Strong understanding of mental health recovery principles and best practices.
  • Excellent communication, interpersonal, and organizational skills.
  • Proficiency in electronic health record (EHR) systems and Microsoft Office Suite.
  • Ability to work independently and as part of a multidisciplinary team.
  • Valid driver's license and reliable transportation.

Nice To Haves

  • Master's degree in social work or a related field.
  • Licensure as a social worker (LSW) or equivalent.
  • Experience working in a residential setting.
  • Bilingual abilities (e.g., Spanish).

Responsibilities

  • Develop, implement, and regularly update individualized service plans (ISPs) in collaboration with residents, treatment teams, and support networks.
  • Serve as the primary point of contact for a caseload of residents, ensuring consistent support, advocacy, and documentation.
  • Coordinate referrals to mental health, medical, vocational, housing, and social services as needed.
  • Assist residents in developing independent living skills, including budgeting, medication management, self-care, and community navigation.
  • Conduct regular one-on-one meetings with residents to assess needs, monitor progress, and support personal goals.
  • Maintain accurate, timely, and confidential documentation of services, assessments, progress notes, and critical incidents.
  • Participate in multidisciplinary team meetings, clinical case reviews, and discharge planning processes.
  • Advocate for resident rights and promote self-determination and recovery-oriented practices.
  • Support crisis intervention efforts, providing de-escalation and coordination with emergency services as needed.
  • Help plan and support resident participation in structured daily activities, community outings, and therapeutic groups.
  • Maintain compliance with all state, federal, and organizational standards and protocols
  • All other duties as assigned.
