Requirements

  • Bachelor’s Degree with major in social work, rehabilitation, sociology, psychology, counseling or human services preferred.
  • 2 years full-time paid professional experience in the Human Services field.
  • Knowledge of psychiatric disorders as defined by APA (DSM IV).
  • Knowledge and familiarity with Rhode Island Mental Health Law.
  • Knowledge of risk assessment instruments and mental status examination.
  • Knowledge of community and state resources.
  • Familiarity with psychotropic medications.
  • Knowledge of laws related to rights, confidentiality, and abuse.
  • Knowledge and skill in crisis theory and crisis intervention techniques.
  • Valid Rhode Island Driver’s License.

Responsibilities

  • Provide primary case management to a selected number of clients and regularly assess client’s level of functioning.
  • Conduct and document psychosocial assessments.
  • Develop and implement MTP (Master Treatment Plans).
  • Promote involvement of family/significant other with treatment planning when appropriate.
  • Assess needs and risk potential for all new and/or established clients who present in crisis.
  • Provide crisis stabilization services and assess for need for hospitalization for at-risk clients.
  • Utilize crisis intervention, stabilization, and counseling techniques.
  • Participate in discharge planning of clients from psychiatric inpatient units as requested by supervisor.
  • Make appropriate and timely referrals to other agencies.
  • Initiate, maintain, update, and monitor master treatment plans.
  • Teach, coach, and assist clients in learning ADLs as needed (e.g., hygiene, shopping, cooking, communication, housekeeping, etc.).
  • Actively participate in team communication processes that ensure appropriate management of cases.
  • Provide consultation and follow-up communication with law enforcement agencies, community agencies, medical facilities, and inpatient units.
  • Supervise residents’ living conditions.
  • Provide meal preparations as outlined by menu and dietary guidelines, especially in the absence of the residential counselor.
  • Transport clients in agency van or own vehicle.
  • Assist clients with financial matters, benefit management, and maintain associated paperwork (e.g., medicaid applications, HUD paperwork, etc.).
  • Monitor or observe medication compliance of clients.
  • Assist client in the development of a support network.
  • Document all assessments, consultations, teaching plans, referrals, treatment plans, and progress of client care.
  • Accurately complete clinical records requirements in a timely and legible manner.
