About The Position

The Resident Care Manager (RCM) - RN role utilizes experienced nursing skills to conduct and coordinate the development and completion of resident assessments (MDS) in accordance with current rules, regulations, and guidelines, including the implementation of CAAs. This role involves coordination with Physicians, healthcare providers, facility inter-disciplinary teams, families, and managing care through the Charge Nurse, Certified Nursing Assistant, and Medication Nurse to best meet the quality of life and care of residents. The facility uses an electronic medical records and electronic charting system.

Requirements

• Licensed RN for the appropriate State.
• Strong assessment and care planning skills.
• 2 – 3 years experience working as an RN.
• Experience working with an electronic medical records and electronic charting system.

Nice To Haves

• Some long-term care experience.
• Strong passion for working with the senior population.
• Experience in Nurse Supervisor roles.

Responsibilities
