Requirements

  • Degree in Nursing from an accredited nursing school.
  • RN required/preferred.
  • Current RN license.
  • Valid CPR certificate.
  • Six months experience in a long-term care environment preferred.
  • Minimum, one (1) year of experience as a supervisor in a hospital, nursing care facility, or other related health care facility preferred.
  • Passionate about providing exceptional, person-centered nursing care.
  • Builds trusting relationships with residents, families, physicians, and team members.
  • Demonstrates excellent communication, organization, and critical thinking skills.
  • Thrives in a collaborative, fast-paced healthcare environment.
  • Has a strong understanding of skilled nursing regulations, nursing practices, and clinical standards.
  • Maintains professionalism, integrity, and compassion in every interaction.
  • Is committed to continuous learning and delivering the highest quality of care.

Responsibilities

  • Develop, implement, evaluate, and update individualized, person-centered care plans.
  • Complete comprehensive assessments for new admissions and significant changes in condition.
  • Partner with physicians, therapists, social services, and interdisciplinary team members to coordinate exceptional resident care.
  • Participate in daily clinical discussions and care conferences to ensure optimal outcomes.
  • Utilize the electronic medical record (EMR-POINT CLICK CARE) to accurately document assessments, interventions, physician orders, and resident progress.
  • Monitor residents' health status and proactively identify changes in condition.
  • Communicate resident condition changes using the SBAR communication process.
  • Review laboratory and diagnostic results and coordinate appropriate follow-up.
  • Implement physician orders and ensure timely completion of treatment plans.
  • Educate residents and families about diagnoses, treatments, medications, and care plans.
  • Provide emotional support and guidance to residents and their loved ones.
  • Ensure medical records are complete, accurate, and survey-ready.
  • Foster a culture of teamwork, compassion, accountability, and outstanding customer service.
  • Maintain reliable attendance and contribute positively to the clinical team.
  • Coordinate discharge planning and smooth transitions to the next level of care.
  • Collaborate with hospitals, physicians, insurance providers, case managers, and community partners to facilitate admissions and discharges.
  • Communicate anticipated discharge needs with the interdisciplinary team.
  • Promote regulatory compliance by following company policies, professional standards, and all applicable federal and state regulations.
  • Perform additional duties as assigned to support resident care and facility operations.
