About The Position

Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. In this home health role, the Registered Nurse guides patients and families through the care continuum by assessing needs, coordinating care, supporting transitions, and managing ongoing outpatient care. The nurse helps ensure patients receive an appropriate level of care, provides education and treatment-decision support, and collaborates with the care team. The schedule is Friday, Saturday, and Sunday.

Requirements

• Current RN license specific to the state or states assigned.
• Current CPR certification.
• At least 1 year of clinical experience as a Registered Nurse.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Refer patients to other disciplines based on identified needs, or document the rationale for not making a referral.
• Promote patient health and independence through teaching and appropriate rehabilitative measures, including instruction in self-care techniques.
• Supervise LPNs and HHAs.
• Complete documentation accurately and on time.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform assigned on-call responsibilities and provide on-call services to patients and families.
• Participate in clinical development and continuing education programs.
• Perform other duties as assigned.
