About The Position
Elara Caring is hiring a full-time Home Health Registered Nurse to provide skilled care to patients in their homes in the Chicopee territory. The RN coordinates an assigned patient caseload, assesses changing needs, and collaborates with an interdisciplinary team to help patients remain safe and supported at home. The role emphasizes clinical judgment, compassionate care, and building meaningful relationships with patients and families. The position is listed as Monday through Friday. Compensation is a guaranteed salary plus a productivity bonus, with pay ranging from $88,000 to $97,000 based on experience. A $5,000 sign-on bonus is offered.
Requirements
- Hold a current Registered Nurse license in the state of practice.
- Graduate from an accredited nursing certificate, diploma, associate, or bachelor's degree program.
- Have at least one year of nursing experience in a clinical care setting.
- Be able to frequently lift, push, pull, and support up to 50 pounds, including assisting with patient positioning, transfers, and equipment.
- Have a valid driver's license and an acceptable driving record, or reliable transportation between patient homes and assigned work locations.

- Home health experience is preferred but not required.
Responsibilities
- Complete admission and ongoing assessments to identify patient needs.
- Provide skilled nursing care according to physician orders and the individualized plan of care.
- Manage and coordinate care for an assigned patient caseload.
- Monitor patient progress, recognize changes in condition, and take appropriate action.
- Teach patients and caregivers to manage care, support recovery, and maintain safety.
- Communicate with physicians and the interdisciplinary team to support continuity of care.
- Maintain accurate clinical documentation in accordance with regulatory requirements.
- Protect patient and staff privacy in accordance with HIPAA.
